- Deductibles from $2,000 to $50,000
Submission Requirements
- Complete ACORD applications (Must include FEIN number, first and last name and phone number of the main contact person)
- Complete program Supplemental Application with a description of operations
- 5-years Currently Valued Loss Runs (GL & WC)
- A detailed description of each loss over $25,000
- WC quote or policy decs including remuneration pages, limits, and Ex Mod factor
- Current and planned project listing including location, the scope of work, and the contract price
- Recently executed Subcontractor Agreement and accompanying Certificates of Insurance, if any work is subcontracted
- Expiring carrier, limits, SIR/deductible, premium, and renewal status
- Resume and/or business plan required if in business less than 3 years
- Description of operations of each Named Insured
- Safety Program information
- If an Umbrella Liability quote is needed for the account, we will also require underlying Auto information (loss runs, quote, declarations page including vehicle schedule)
